Estonia is widely recognized as Europe’s pioneer in digital health, with a national digital identity system that has been in use for over 20 years and a universal electronic health record (EHR) system. The region stands out for its 99% digitized health data and a massive biobank covering 20% of the adult population, which far exceeds the European average of 5%. Renowned for its cutting-edge data infrastructure, health technology now accounts for 23% of all Estonian startups specializing in advanced technologies.

Estonia’s main strengths lie in its large-scale digital integration, with 99% of prescriptions issued electronically and 97% of discharge summaries integrated into the National Health Information System (HIS). The HIS stores over 40 million health records, including not only the aforementioned discharge summaries but also consultation summaries, lab reports, and referrals to specialists. The use of KSI Blockchain technology ensures top-tier data integrity and security. To protect sensitive data, Estonia uses KSI Blockchain technology, which guarantees data integrity and prevents tampering. Each electronic health record is securely linked to a national electronic identification system, thereby balancing accessibility with a high level of security.

Estonia’s integrated e-health ecosystem demonstrates how technology can improve care, optimize outcomes, and empower patients. As a pioneer in this field, Estonia offers a model for the digital transformation of healthcare on a global scale.

Today, the main challenge is the need to accelerate the clinical, regulatory, and commercial validation of new technologies. To address this challenge, Estonia has established a national experimentation framework and a 12-step roadmap for innovation in health technologies. Collaboration on AI-based regulatory solutions and hybrid financing models are key priorities for the 2025–2030 period.

The Estonian ecosystem is a well-balanced environment comprising 4,222 companies in the healthcare sector, generating more than 1.5 billion euros in added value. It is supported by well-established ecosystem support organizations such as Health Founders Estonia, Tehnopol, and the Tartu Biotechnology Park.

Estonia is a leader in cross-border digital services, with an electronic prescription exchange system operational in nine countries through the MyHealth@EU initiative. The country ranks first in the Bertelsmann Foundation’s 2024 Digital Health Index. Estonia actively participates in international strategic groups such as the eHealth Network, HL7, and EARTO WG, and regularly hosts Estonian Healthtech Week to foster global partnerships.

Key initiatives and active partnerships include:

  • International research and funding: The Tehnopol HealthTech ecosystem actively participates in European projects such as Horizon, Interreg, and I-3.
  • Tehnopol’s global reach: The Tehnopol HealthTech community maintains relationships with 500 international organizations worldwide and helps Estonian companies expand into more than 15 target markets each year. Our export and collaboration activities take us beyond borders, particularly to other markets. Last year, for example, we conducted a visit to Germany in collaboration with the German-Baltic Chamber of Commerce in Estonia, Latvia, and Lithuania (AHK), as well as visits to the U.S. and U.K. markets as part of the Interreg Health3-2-1 program.

Throughout the year, we represented the Tehnopol ecosystem and established partnerships in Norway, France, Germany, India, Belgium, the United Kingdom, the United States, Finland, Spain, Australia, and Canada (Nova Scotia).

Key areas of focus: The Tehnopol Health Tech community focuses on the following areas: business development, exports, attracting investment, cross-sector (public/private) cooperation, and contributing to the national strategy.

  • Access to the ecosystem’s international network;
  • Partner matching for clinical studies, trials, and pilot projects;
  • Access to private and public funding opportunities; assistance in securing investments; Mentoring and visits by high-level delegations. Export opportunities through EU Horizon projects, Interreg, and I-3 – cross-border collaboration and development opportunities;
  • Marketing and communication support – sharing success stories with over 5,000 stakeholders via newsletters in Estonia and the ECHAlliance network;
  • Collaboration with the public sector on policy development and implementation;
  • Opportunities to participate in various international projects.

Key Achievements / Projects:

The Tehnopol HealthTech community has won several international and national awards, including the “European Cluster Excellence Gold Label” and the Estonian Business Cooperation Award in 2023.

Our joint project DIGILUNG, carried out with ATRIDE, moveUP – digital therapies, Cognuse, and the Vrije Universiteit Brussel, was named one of the four best I3 (Interregional Innovation Investments) projects by the European Broadband Competence Offices (BCO) network. This prestigious recognition from the European Commission highlights DIGILUNG as an example of best practice, illustrating how well-implemented I3 projects can drive innovation across Europe.

Other flagship projects include Health3-2-1 (2024–2027) and the Welfare Technology program.

Role in regional innovation: The Tehnopol HealthTech community (formerly the Connected Health Cluster) is the largest health technology ecosystem in the Baltic states and a key implementation partner for Estonia’s roadmap for innovation in health technology. We connect 105 Estonian and international organizations, including global pharmaceutical companies, startups, technology firms, biotech companies, research institutes, public sector representatives, and international contacts within the ecosystem.
